Prior to your first appointment your counselor will send you our intake forms via our electronic records system: Patient Ally. We will need your full name, cell phone number, and email address to set you up with that system. Intake forms will need to be signed prior to starting services.
The counselor or staff you schedule with will give you the code to the door for when you arrive. We do not currently have a receptionist, so feel free to find a seat in our waiting room when you arrive. At our 805 Liberty Street location, the waiting room is located to the right at the top of the short flight of stairs. At our 867 Liberty location, the waiting room is right inside the front door and also includes the conference room with the large table. Restrooms in the 805 Liberty location are located down the left hallway. Your counselor will come out to meet you at your scheduled appointment time.
Ongoing appointments with your counselor will be scheduled at mutually agreed upon times. Many of our clients have a standing weekly appointment at the same time on the same day each week. Talk to your counselor about how they manage their schedule.
